Enroll at St. Marys City Schools

New Student Enrollment
Welcome to St. Marys City Schools!  In order to register your child(ren) to attend St. Marys City Schools, please complete the following steps.
  1. Complete the online enrollment forms on FinalForms, an online enrollment system.  The online forms in work best on desktop computers, laptops, chromebooks, or tablets.  If you need access to a device,  you can complete the online enrollment forms on a device in the lobby of the District Service Center.  
  2.  The following documents are required for each child: 
    1. Birth Certificate (hospital certificate, front and back, is accepted)
    2. Immunization Records 
    3. Proof of Residency 
    4. Custody Papers (if applicable)

  3. These documents may be submitted electronically through FinalForms or by scanning them and attaching them in an email to [email protected]. 

Once you have completed the online registration, you will be contacted regarding your student’s official start date and transportation information.  Please feel free to contact the DSC at 419-394-7278 if you have any questions.
Scan Documents
Don't have access to a scanner?  These google apps allow you to scan documents with your phone!
For all devices
1. Go to your google photos or download the Google Photos and Photo Scan by Google apps
2. Using the Photo Scan app, take a picture of the document and follow the steps for successful scanning.  
3. Repeat step 2 for each document
4. In the Google Photos app, select the images you just scanned and select “share” (square icon with up arrow or a triangular shape with circles in the corners).  If asked, please select "large" or "best" quality for each photo.  
5. Email to: [email protected] with subject line of Enrollment documents - First initial Last initial (ie. Enrollment documents - JV)

St. Marys City Schools uses FinalForms for new student registration and to maintain accurate student information (such as phone numbers, addresses, and medical information).  Parents are asked to review/ update this information annually and as changes occur (new phone number, new address, etc.).
